Vendor Agreement

We bring awareness and support for Veterans suffering from Post-Traumatic Stress and their families who support them

In consideration of the sum of ONE ($1.00) DOLLAR and other good and valuable consideration,
OPERATION-INITIATIVE FOUNDATION, (hereinafter called the “OIF"), a domestic not-for-profit corporation, having its principal place of business at 535 South Broadway, Suite 215B/C, Hicksville, New York 11801 and

(hereinafter called the indemnitor/vendor"),

whose Office is at:

the receipt of which is hereby indemnify and hold harmless Operation-Initiative Foundation of and from any and all manner of claims, demands, suits, actions or judgments including attorney’s fees and costs which hereafter may be asserted, instituted or recovered against OIF due to any action or negligence by any action or negligence by any subcontractors or employees or agents of the Indemnitor/vendor, pursuant to the Participation Agreement between OIF and Indemnitor/vendor and any participant or family member or Veteran seeking assistance from OIF for the period up to the execution hereof, and hereafter included within the foregoing term "agent" shall be the claim of any governmental authority or union.

Operation-Initiative Foundation to the Indemnitor/vendor in writing, addressed in care of indemnitor/vendor, by Certified Mail, Return Receipt Requested, and the said Indemnitor. Vendor shall have the sole right and obligation to defend or settle the same at its own sole cost and expense.
